Lisa Miller, MD

Dr. Lisa is an Indiana-native who grew up in Fort Wayne. Dr. Miller decided to become a doctor at the age of 8 and after graduating from New Have High School her junior year (and being a self-proclaimed studious “nerd”), she attended Purdue University, graduating with her B.S. degree in Health Sciences.

She then attended I.U. Medical School and completed her Family Practice Residency at Methodist Hospital, in Indianapolis in 1989. Since that time, Dr. Miller has been in private Family Practice and is an active board-certified member of the Academy of Family Physicians. She is also am a member of the American College of Sports Medicine and Nutritional Society.

Amy Nicely, NP

Amy began her work in healthcare in 2003 as a Nurse at Methodist Hospital after completing her Bachelors of Science in Nursing at Anderson University. She completed her Masters of Science in Nursing from the University of Indianapolis in 2006 and has been practicing as a Certified Family Nurse Practitioner since that time. For her first 8 years of practice as an NP, she practiced Internal medicine at a large hospital yet always felt a longing to practice a more preventative approach.

Amy’s own health needs and those of my family led me to Excell For Life as a patient and wanted to help patients the same way Functional Medicine helped her. She soon joined the Excell For Life team of Providers in September of 2014 and completed her Institute for Functional Medicine certification training in 2017.

Nathan Bleeke, DO

Nathan received his education at Purdue University in West Lafayette and graduated in 2013 with two Bachelor’s Degrees in Chemistry and Environmental Geoscience.  Medicine has always been of interest and he was soon accepted to Marian University’s College of Osteopathic Medicine after his undergraduate at Purdue.

Since graduating from Medical School in the summer of 2018, Nathan’s osteopathic training has had a large influence on how he practices as a physician. The osteopathic model is based on incorporating a mind, body and spirit approach as well as considering the body’s natural ability to heal itself. His personal interests in medicine include diabetes management, cardiology, women’s health, pediatrics, and osteopathic manipulation.

Tara Fox, DNP

Tara earned her undergraduate degree in Biological Sciences from Virginia Tech before completing her R.N. and Master of Science in Nursing at Vanderbilt University in 2010.

She began her career in Washington, D.C., working in a specialty clinic focused on infectious disease and post-infectious syndromes. During her six years there, she collaborated with healthcare providers across the country and abroad, combining her knowledge of child development with evidence-based practice to help improve outcomes for patients living with chronic illness related to infection.

After her time in D.C., Tara worked in central and northern Virginia at a pediatric urgent and emergent care clinic.

In 2020, Tara earned her Doctor of Nursing Practice (DNP) degree from Georgetown University, with her doctoral research focused on improving screening for PANDAS, a rare disorder that can follow strep infections. She has since been published and invited to speak on the recognition and treatment of the condition.

Amanda Miller, PA-C

Amanda is an Indianapolis native and loves serving her community. She is a proud graduate of DePauw University where she cheered, was a member of Kappa Alpha Theta and graduated with degrees in Biology and Spanish.   For the past ten years she has worked as a PA in family medicine and urgent care and enjoys providing healthcare to people of all different backgrounds.  Her own healthcare needs led her to Excell for Life and she has been so inspired by how the providers address the root cause of so many conditions and focus on nutrition, lifestyle and mental/ spiritual health.  She is so excited to begin working at Excell and prays she can help empower patients to achieve their health goals, heal from chronic illness and optimize their quality of life.  

Amanda lives with her amazing husband Blake and their 3 young children in Carmel.  They love spending time outdoors and soaking up time with their little ones.  She loves to spend time with friends, be active, cook, and is always reading a new book!  They have been members of Grace Church the past 12 years and have served in various ministries there.  She is extremely grateful for this opportunity and can’t wait to meet you and your family!

Abigail hendrickson, PA-C

Abby began her academic journey at Purdue University, where she studied Biochemistry, before continuing her education at Butler University to become a Physician Assistant. With nearly a decade of experience working in a large hospital system, Abby has gained valuable expertise in both urgent care and as an internal medicine hospitalist. Throughout her career, she has had the privilege of helping countless patients navigate both physical and mental health challenges. Abby is passionate about empowering patients through education and prevention, and she is committed to making the journey to optimal health a positive and hopeful experience for all. 

Abby is excited to join a team that shares her values of faith, family, and root cause medicine. Outside of work, she is happily married to her high school sweetheart, and together they have three wonderful children. As a family, they enjoy a variety of activities, including outdoor adventures, movie nights, and road trips to the beach. Abby loves all things floral and her garden grows larger every year!
